The vitality of a community has an influence on the health of the people who live, work, learn and play there. A healthy community is one in which multiple sectors collaborate to improve the conditions that influence our overall health and well-being. Given one of the fundamental core values of the healthy communities approach is capacity building and empowerment of individuals, organizations and communities, local community hubs play a critical role working directly toward optimizing health and well-being for all, especially the most vulnerable.
This map provides an overview of select community innovators and leaders working in community health hubs, as well as with the homeless and addiction across the country (British Columbia, Saskatchewan, Ontario, and Nova Scotia). One of our objectives is to document the enhanced social outcomes of these centres by bringing together leaders from across the country for a series of e-Dialogues during the fall and winter of 2018/2019.
